Behavior notes that actually say something.

Staff tap a few plain answers on a phone. Divinity writes a signed note that says what happened, for every shift and every client, in one place you can read. And the questions get smarter about each client every week.

Goes in the note

The client asked the same question again and again.

A real question card from Divinity. Tap the answers.

The notes matter. Writing them is the problem.

  • State and MCOs require them.
  • Staff belong with clients.
  • Staff see their good work.

Nothing happened. Client was fine.

How it works

  1. For the first 7 to 10 days, Divinity learns each client

In the first week, staff answer a quick question or two about the person: what calms them, what sets them off, what they are good at. The people who know the client best fill in the picture, one tap at a time.

  2. Staff tap through the shift on their phone

Staff record a client whenever something worth noting happens, right then, in about a minute. The more they record, the more Divinity learns. Every question uses plain words and every answer is a tap. Each tap adds one sentence to the note. Staff read it, sign, and they're done.

  3. Then the questions adapt, and the records get better

After the first week or so, the questions change. The ones that matter for this client move to the front. The ones that never happen fall away. Fewer taps, notes that say more, and a picture of each client that gets clearer every week.

The note nobody had to write

Every sentence below came from a tap. Staff read it over, can adjust it, then sign.

Signed records
Each note carries the signature of the person who wrote it and the time they signed.
Edit history
Every change is recorded with who made it and when.
Late entries marked
A note entered after the fact is marked as late, with the time it was actually entered.

Questions owners ask

Do staff need training?

Very little. The app asks one thing at a time in plain words, and every answer is a tap. Staff never have to write the note themselves.

What phone do staff need?

The one they already have. Divinity works in the browser on the phones and computers you already have, so there's nothing to install.

Can I see what staff submit?

Yes. As the owner you can read every signed note, see who wrote it and when, and see each client's patterns over time.

What happens after the trial?

After 14 days you choose whether to subscribe, from the billing page in the app. Starter is $79 per month per facility with 3 client seats included, then $10 per month for each additional active client. Nothing is charged during the trial.

Can I cancel?

Yes. You manage billing yourself in the app, including canceling. There's no one to call.

Is it only for Washington?

It's built for Washington adult family homes and behavioral health facilities, and it works anywhere.

Where is my data stored?

On Google Cloud, in the United States. It's sent over encrypted connections and stored securely. Everyone signs in with their own account, and every change is recorded.

Does it replace my existing forms?

It replaces the behavior note staff write for each client every shift: the signed note Divinity writes is that record. Keep any other forms your facility uses until you're sure they're covered.
